NHHA’s vision for integrated whole-person care

National Healthcare and Housing Advisors (NHHA) set out to bridge healthcare, housing, and social services under a coordinated system. Through its Community Care Campus model, NHHA brings together healthcare, housing, and social services under one roof — a first-of-its-kind approach that is reshaping how people with complex needs experiencing homelessness receive care and support.

Their mission: Bringing together medical, behavioral, and housing services to deliver whole-person care.

Our goal from day one was to stop working in silos and start working as a single system for the people we serve.

NHHA’s EHR faced limitations in managing housing, social services, and care coordination workflow needs. Teams filled gaps with spreadsheets and workarounds, slowing coordination, creating silos in care and leaving leadership without visibility.

Driving adoption through the Superuser Program

NHHA understood adoption was critical.
The answer was a Superuser Program that turned adoption into a shared mission, led by key champions located at each clinical site. This approach transformed training from static instruction to collaborative problem-solving.

The impact and looking ahead

NHHA has developed the infrastructure to see across programs in real time, streamline workflows, and harness data for compliance and population health—paving the way for NHHA to scale its integrated Management Services Organization (MSO) model and drive better outcomes.

Next, NHHA and Chorus will expand their work by advancing a standardized care plan across programs, strengthening analytics for high-risk populations, and adding medication and vitals management to further support value-based care at scale.

Now, leadership can see what’s happening in real time—across all programs. That visibility didn’t exist before.
